Primary Care Nurse Practitioner

📋 Description

• Provide ongoing primary and preventive care to older adult patients
• Manage and grow a dedicated patient panel within assigned communities
• Deliver relationship-driven, patient-centered care in community settings
• Collaborate with interdisciplinary clinical and operational teams
• Support preventative care, chronic disease management, and care coordination
• Maintain accurate EHR documentation and ensure coding specificity

🎯 Requirements

• Active, unrestricted Nurse Practitioner license
• Board Certification: NP (AANPCB or ANCC)
• Active DEA license and controlled substance registration
• 3+ years of clinical experience as an NP
• Basic Life Support (BLS) certification
• Experience with EHR documentation (athenahealth preferred)
