Primary Care Physician, Pod Leader

📋 Description

• Deliver in-person assessments with the IDT to create personalized care plans
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds; focus on comprehensive care with ~8 patients/day
• Value-based PACE model guiding holistic care
• 8am–5pm, Monday–Friday; includes home visits
• Interdisciplinary team collaboration to support vulnerable seniors

🎯 Requirements

• ACGME-accredited residency completed
• Master's or Doctorate Degree
• Active state license without restrictions
• Board Certification preferred
• State CDS and DEA registrations
• CPR and First Aid certification within 180 days
