Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide continuous medical coverage and in-person assessments (initial, semi-annual, unscheduled).
• Collaborate with Physicians and IDT to develop care plans and coordinate care.
• Assist in maintaining care in skilled nursing facilities and residences with regular visits.
• Review outcomes data to improve practices and policies.

🎯 Requirements

• Graduation from an accredited school of nursing.
• NP or PA certification with prescriptive authority.
• RN state license.
• Minimum two years with frail/elderly populations.
• Knowledge of chronic care and geriatric issues.
• Able to rotate night and weekend call.
