Provide continuous medical coverage and in-person assessments (initial/semi-annual/unscheduled).
Collaborate with physicians and IDT to develop care plans and coordinate care.
Maintain participant care in skilled nursing facilities and residences with regular visits.
Review outcomes and utilization data to establish best practices and policy improvements.

🎯 Requirements

Graduation from an accredited school of nursing
NP or PA with prescriptive authority
RN license
Minimum 2 years of experience with frail or elderly populations
Knowledge of chronic care and geriatric issues
Reliable transportation
Able and willing to rotate night and weekend call
