Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Serve a smaller patient caseload to build long-term relationships and value-based care
• Collaborate with Physicians and IDT to develop participant care plans and coordinate care
• Provide continuous medical coverage and in-person assessments (initial, semi-annual, unscheduled)
• Assist in maintaining participant care in skilled nursing facilities and residences with regular visits
• Review outcomes and utilization data to drive best practices and policy improvements
• Work within a collaborative IDT and WelbeHealth team to support holistic care

🎯 Requirements

• Graduation from an accredited school of nursing
• NP or PA with prescriptive authority
• RN state license
• 2+ years experience with frail or elderly populations
• Knowledge of chronic care and geriatric issues
• Able and willing to rotate night and weekend call
