Provide continuous medical coverage to participants and in-person assessments (initial, semi-annual, unscheduled)
Collaborate with Physicians and the IDT to develop care plans and coordinate care
Maintain participant care in skilled nursing facilities and residences with regular visits
Review outcomes and utilization data to drive practice improvements

🎯 Requirements

Graduation from an accredited school of nursing
Nurse Practitioner or Physician Assistant with prescriptive authority
RN license
Minimum of two years' experience with frail or elderly populations
Knowledge of chronic care and geriatric issues
Available for rotating night and weekend call
