Provide ongoing medical coverage and in-person assessments (initial, semi-annual, unscheduled)
Collaborate with physicians and the IDT to create care plans and coordinate care
Maintain care in SNFs and homes with regular visits as needed
Review outcomes data to improve best practices and policy

🎯 Requirements

Graduation from an accredited nursing program and RN license
NP/PA certification with prescriptive authority
At least 2 years experience with frail or elderly populations
Knowledge of chronic care and geriatric issues
Reliable transportation
Willing to rotate night and weekend calls
