Provide continuous medical coverage to participants with in-person assessments
Collaborate with IDT to develop care plans and coordinate discharge planning
Support participants in skilled nursing facilities and homes with regular visits

🎯 Requirements

Graduation from an accredited school of nursing
NP or PA with prescriptive authority
RN state license
Minimum of two years with frail/elderly populations
Knowledge of chronic care and geriatric best practices
Reliable transportation; willing to rotate nights/weekends
