📋 Description

• Oversee critical PACE functions: nutrition, social work, day center, personal care, scheduling, and
• Develop and maintain a high-functioning Interdisciplinary Team (IDT) and coordinate
• Ensure adherence to SOPs and regulatory requirements; supervise and mentor staff; optimize
• Collaborate to achieve high participant outcomes with services including home care, home health
• Oversee site operations, supplies, equipment, maintenance, and vendor oversight.

🎯 Requirements

• Bachelor's degree in health admin/health care management/gerontology/social work or related field
• Professional experience in nursing, social work, psychology or rehab with frail elderly home-based
• Minimum seven years in relevant roles, with at least three in supervisory capacity managing a
• Multi-disciplinary management experience (nursing, social work, rehab); value-based care preferred.
• CPR and First Aid certification within 180 days of hire (certification on hire preferred).
