Psychiatric Mental Health Nurse Practitioner (PMHNP) Team Lead

📋 Description

• Provide psychiatric consultation, evaluation, counseling, and treatment with the IDT.
• Collaborate with PACE Physicians and IDT on discharge planning.
• Refer participants to psychiatrists as indicated; manage care in nursing homes.
• Provide clinical leadership and mentorship to psychiatric NPs and behavioral health team.
• Serve as first point of contact for complex cases and pre-enrollment decisions.
• Support onboarding and ongoing training; participate in quality improvement.

🎯 Requirements

• MS in nursing with a psychiatric/mental health emphasis.
• Certified as a Psychiatric Mental Health Nurse Practitioner.
• Active PMHNP license without restrictions.
• State CDS Certification, NPI and DEA License.
• Medicare and Medi-Cal enrollment eligibility required.
• 4+ years experience with frail/elderly; 2+ years in PACE preferred.
