• Build and maintain trusted relationships with partner quality improvement teams and key clinical
• Use ACG and population health analytics platforms to stratify patient populations, identify care
• Develop and maintain baseline reporting for ACO quality measures across partner populations
• Analyze quality and population health data to identify trends, risks, and opportunities

🎯 Requirements

• Active Registered Nurse (RN) license is required.
• 5+ years of clinical experience, including meaningful experience in population health, quality
• Strong subject matter expertise in ACO quality measures and population health quality improvement
• Hands-on experience with the Johns Hopkins ACG platform or a comparable risk-stratification or
• Strong analytical instincts and the ability to investigate healthcare datasets, identify meaningful
• Demonstrated ability to build trust and deliver results with external partners, including senior
