Conduct home health nursing assessments for equipment needs and goals
Actively participate in IDT meetings to develop participant care plans
Deliver and document nursing interventions per care plans
Instruct participants and families on disease processes and self-care
Track scheduling with the Home Health Manager
Collaborate with physicians and IDT for smooth care transitions between settings

🎯 Requirements

Graduate of an accredited school of nursing
Unencumbered RN license required
1 year nursing experience with frail elderly; home health preferred
Reliable means of transportation
Must obtain CPR and First Aid certification within 180 days; preferred on hire
