📋 Description

• Provide home health visits to clients in designated Somerset/Union/Middlesex Counties.
• Perform nutrition assessments and develop individualized care plans.
• Deliver medical nutrition therapy to support disease management and overall health.
• Counsel clients and families on nutrition based on health needs and goals.
• Educate clients, caregivers, and clinical team on nutrition management.
• Translate evidence-based nutrition into practical home strategies.

🎯 Requirements

• Bachelor's degree or higher in nutrition, dietetics, or related field.
• Current Registered Dietitian (RD/RDN) credential.
• Meet NJ requirements to practice as a Registered Dietitian.
• Minimum of two years of RD experience in healthcare.
• Home health experience preferred but not required.
• Strong communication and interpersonal skills; ability to work independently.
