📋 Description

• Provide home health visits to clients throughout designated territories.
• Complete comprehensive nutrition assessments and develop nutrition care plans.
• Provide medical nutrition therapy to support disease management and overall health.
• Counsel clients and families on nutrition recommendations based on health needs and goals.
• Educate clients, caregivers, and clinical team on nutrition and dietary management.
• Translate nutrition recommendations into practical strategies for home use.

🎯 Requirements

• Bachelor's degree in nutrition or related field from an accredited institution.
• Current Registered Dietitian (RD/RDN) credential.
• Must meet New Jersey requirements to practice as an RD.
• Minimum of two years of recent RD experience in a healthcare setting.
• Home health experience preferred but not required.
• Strong communication, education, and interpersonal skills.
