📋 Description

• Respond to after-hours patient needs within coverage area
• Perform Start of Care (SOC) admissions as needed
• In-home skilled nursing visits and care delivery
• Monitor patient conditions and communicate changes
• Complete SOC or SOC-related documentation
• Travel within Somerset, Union, Middlesex, Monmouth Counties

🎯 Requirements

• Current RN license in New Jersey
• Minimum one year of home health nursing experience
• Experience with OASIS documentation and SOC visits
• Strong clinical decision-making and communication skills
• Comfort with EMR; HCHB is a plus
• Reliable transportation for county travel
