Registered Nurse, Evenings

📋 Description

• Per Diem On-Call RN for evening home health coverage
• Provide in-home skilled nursing visits for adult/geriatric patients
• Conduct Start of Care admissions as needed
• Deliver patient-centered care with support from clinical team
• Schedule includes Mon-Thu, 5:30 PM–8:30 PM, on-call during hours
• Travel within Somerset, Union, Middlesex, and Monmouth Counties

🎯 Requirements

• Current RN license in New Jersey
• Minimum 1 year home health nursing experience
• Experience with OASIS documentation and SOC visits preferred
• Strong assessment and clinical decision-making skills
• Proficient with EMR; HCHB experience a plus
• Reliable transportation to cover assigned counties
