Follow the prescribed care plan to meet patient needs
Perform home visits in designated geographic territories
Administer meds, wound care, and treatments
Monitor client conditions and report changes to Clinical or Client Services Manager
Document orders and follow up; ensure proper documentation
Conduct client assessments and care coordination

🎯 Requirements

Current RN license in Delaware
At least 1 year of recent clinical (medical/surgical) experience
Graduation from an accredited nursing program
Ability to work independently and manage time effectively
Strong interpersonal skills
Solid computer skills; EMR experience preferred
