Follow a designated care plan per patient needs.
Make home visits to clients in Smyrna, Clayton, and Dover area.
Administer medications, wound care, treatments, and procedures.
Monitor client conditions and report changes to Clinical or Client Services Manager.
Document doctors' orders and perform client assessments.
Case management and coordination; document care on a touch pad tablet.

🎯 Requirements

A current DE RN license.
Minimum one year of recent clinical (medical/surgical) experience.
Home care experience preferred, but not required.
Graduation from an accredited nursing program (transcript or diploma).
Ability to work independently and manage time.
Solid computer skills with EMR experience preferred.
