Registered Nurse - Home Health Visits

📋 Description

• Conduct home health visits for adult and geriatric patients in MD
• Follow designated care plans and document care in EMR
• Administer medications, wound care, and prescribed treatments
• Monitor patient conditions; report changes to Clinical or Client Services Manager
• Perform case management and coordinate with care team
• Travel to cases as assigned within service area

🎯 Requirements

• Maryland RN license (current)
• 1+ year clinical (medical/surgical) experience
• Prior home care experience preferred
• Graduation from accredited nursing program
• Ability to work independently and manage time
• EMR experience or strong computer skills
