Registered Nurse-Home Health Visits

📋 Description

• Follow designated care plans for each patient.
• Make home visits to clients in designated territories.
• Administer medications, wound care, and treatments.
• Monitor client conditions; report changes to supervisor.
• Follow up with, execute, and properly document doctors' orders.
• Perform client assessments and case management as needed.

🎯 Requirements

• A current PA RN license.
• Minimum 1 year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience preferred, but not required.
• Graduation from an accredited nursing program (diploma or transcript).
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; solid computer skills; EMR experience preferred.
