📋 Description

• Use head-to-toe assessment skills to spot subtle changes in patient condition
• Acute and post-acute care experience (CHF, COPD, wounds, IV therapy)
• Triage and manage urgent clinical situations independently at home
• Perform medication reconciliation, teaching, and safety monitoring
• Conduct comprehensive wound assessment, care, and documentation
