Registered Nurse-Home Health Visits

📋 Description

• Follow a designated care plan per patient needs
• Travel to home visits in designated geographic areas
• Administer medications, wound care, treatments, and procedures
• Monitor client conditions; report changes to the Clinical or Client Services Manager
• Follow up with, execute, and document doctors' orders
• Perform client assessments and coordinate care

🎯 Requirements

• Active Pennsylvania RN license
• Minimum 1 year recent clinical (medical/surgical) experience
• Prior home care experience preferred but not required
• Graduation from an accredited nursing program (transcript or diploma)
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; EMR experience preferred
• Ability to travel to assigned cases
