📋 Description

• Head-to-toe assessment to detect subtle changes in condition
• Acute and post-acute care (CHF, COPD, wounds, IV therapy, post-surgical care)
• Triage and independently handle urgent clinical situations at home
• Medication reconciliation, teaching, and safety monitoring
• Wound assessment, care, and documentation

🎯 Requirements

• Current PA nursing license in good standing
• Graduation from accredited nursing program
• 1 year adult/geriatric care in relevant settings
• Independently work and escalate when needed
• Real-time clinical decisions without on-site support
• Efficient point-of-care documentation and use of technology
