Registered Nurse, Home Health Visits

📋 Description

• Provide skilled nursing and rehabilitative care to home health clients in the Harrisburg area.
• Perform head-to-toe assessments and identify changes in patient condition.
• Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and independently handle urgent clinical situations in the home.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Complete wound assessments, care, and documentation in the point-of-care system.

🎯 Requirements

• A current nursing license in good standing (state of Pennsylvania).
• Graduation from an accredited nursing program with transcript/diploma.
• At least one year of hands-on medical experience with adult/geriatric populations in relevant
