Follow a designated care plan in patient's needs
Make home visits to clients in designated geographic territories
Administer medication, wound care, treatments, and procedures
Monitor clients' conditions; report changes to Clinical or Client Services Manager
Follow up with, execute, and document doctors' orders
Perform client assessments and care coordination

🎯 Requirements

A current license as a Registered Nurse in Pennsylvania
A minimum of one year of recent clinical (medical/surgical) experience
Graduation from an accredited nursing program
Ability to work independently and manage time effectively
Strong interpersonal skills
Solid computer skills; EMR experience preferred
