Registered Nurse-Home Health Visits

📋 Description

• Provide home health nursing visits in designated geographic area (Schuylkill County).
• Follow care plans and administer medications and wound care per patient needs.
• Monitor client conditions and report changes to the manager.
• Document observations, interventions, and evaluations using EMR/tablet.
• Coordinate case management and follow up on doctors' orders.
• Deliver 1:1, compassionate care with strong clinical judgment.

🎯 Requirements

• Current PA Registered Nurse license.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduate of an accredited nursing program; ability to work independently.
• Strong interpersonal skills and solid computer/EMR skills.
• Willingness to travel to cases as assigned.
