Registered Nurse - PST

📋 Description

• Act as a liaison for your patient panel of medically stable patients.
• Monitor and triage lab orders/results; coordinate care with internal/external providers.
• Manage the RN phone line for care coordination with external providers.
• Follow triage protocols and respond to families' questions.
• Provide families with condition-specific education and medical management support.
• Coordinate care with hospitals for medical stabilization admissions and transitions.

🎯 Requirements

• Current and unrestricted RN license in at least one US state; compact license preferred.
• 2+ years RN experience in clinic, homecare, ambulatory, hospital or inpatient setting.
• Experience coordinating care with multiple providers and patient support systems.
• Ability to manage multiple requests and prioritize accordingly.
• Excellent communicator with confidentiality adherence.
• Ability to work autonomously within outlined procedures and protocols.
