📋 Description

• See a client through care needs from hospital discharge to safe home life.
• Deliver 1:1 skilled nursing and support per care plan (medication, wound care).
• Perform assessments, educate patients and families, and monitor condition.
• Calm, reassuring presence with active listening and strong communication.

🎯 Requirements

• Active RN license in Pennsylvania.
• One year of clinical experience strongly preferred.
• Previous home health experience welcomed; training provided.
• Ability to travel to cases as assigned.
• Strong communication and compassionate patient care.
