Registered Nurse, RN, Home Health

📋 Description

• Follow designated care plans per patient needs
• Home visits to clients in assigned geographic areas
• Administer meds, wound care, treatments
• Monitor and report changes to Clinical or Client Services Manager
• Document doctors' orders; follow up and execute
• Perform client assessments as needed
• Case management and coordination
• Document observations and interventions on touchpad tablet

🎯 Requirements

• Current RN license in PA
• At least 1 year of recent clinical (medical/surgical) experience
• Home care experience preferred but not required
• Graduation from an accredited nursing program (transcript or diploma)
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; EMR experience preferred
• Ability to travel to cases as assigned
