Registered Nurse, RN - Home Health

📋 Description

• Provide skilled nursing and rehab care in clients' homes.
• Conduct head-to-toe assessments and detect condition changes.
• Manage acute and post-acute needs (CHF, COPD, wound care, IV therapy).
• Triage and independently manage urgent clinical situations at home.
• Perform medication reconciliation, teaching, and safety monitoring.
• Complete wound assessment, care, and documentation.

🎯 Requirements

• Massachusetts nursing license in good standing.
• Graduation from an accredited nursing program.
• At least 1 year of hands-on adult/geriatric experience in relevant settings.
• Medicare Certified Home Health setting experience with OASIS documentation.
