Registered Nurse, RN, Home Health Visits

📋 Description

• Follow a designated care plan per patient needs
• Make home visits to clients in designated territories
• Administer medications, wound care, treatments, and procedures
• Monitor clients' conditions; report changes to the Clinical or Client Services Manager
• Follow up with, execute, and document doctors' orders
• Document observations and evaluations using a touch pad tablet

🎯 Requirements

• A current DE RN license
• Minimum of one year of recent clinical (medical/surgical) experience
• Prior home care experience required
• Graduation from an accredited nursing program (transcript/diploma)
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; EMR experience preferred
• Ability to travel to cases as assigned
