Registered Nurse, RN, Home Health Visits

📋 Description

• Follow a designated care plan per patient needs.
• Make home visits to clients in Erie County.
• Administer meds, wound care, treatments, procedures.
• Monitor client conditions and report changes promptly.
• Document observations and interventions in EMR on a tablet.
• Coordinate case management and care services.

🎯 Requirements

• Active PA Registered Nurse license.
• Minimum 1 year of recent clinical (med/surg) experience.
• Home care experience preferred but not required.
• Graduation from accredited nursing program.
• Ability to work independently and manage time.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
