Registered Nurse, RN, Home Hospice

📋 Description

• Follow designated care plan per patient needs
• Conduct home visits to clients in designated territories
• Administer medication, wound care, treatments
• Monitor client conditions and report changes
• Document doctor's orders and follow-up actions
• Perform client assessments and care coordination

🎯 Requirements

• Current SC RN license
• Minimum 1 year recent medical/surgical clinical experience
• Graduation from accredited nursing program
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; EMR experience preferred
