📋 Description

Make home visits to clients in designated territories.
Provide meds, wound care, treatments, and comfort care.
Monitor client conditions and report changes to the care team.
Follow doctors' orders and properly document care.
Occasional on-call after hours and weekends.
Work with a multidisciplinary team to deliver home-based care.

🎯 Requirements

Active and current NJ RN license.
One year hospice experience preferred.
Home health experience welcomed; training provided.
Ability to travel to assigned cases.
Strong communication and assessment skills.
