Registered Nurse, RN- Hospice, Full time

📋 Description

• Perform home visits to clients in designated geographic territories.
• Administer medication, wound care, treatments and procedures, and comfort care.
• Monitor client conditions and report changes to Clinical or Client Services Manager.
• Follow up with, execute, and document doctors' orders.
• Conduct client assessments as necessary.
• Occasional on call after hours and/or weekends.

🎯 Requirements

• A current RN license in VT and NH.
• Minimum of one year of recent, verifiable clinical experience.
• Prior home care or hospice experience preferred but not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
