Make home visits to clients in designated geographic territories.
Administer medications, perform wound care, treatments and comfort care as assigned.
Monitor client conditions and report changes to the Clinical or Client Services Manager.
Follow up with, execute, and document doctors' orders properly.
Perform client assessments as necessary.
Occasional on-call after hours and/or weekends.

🎯 Requirements

A current NH RN license.
Minimum of one year of recent, verifiable clinical experience.
Home care or hospice experience preferred but not required.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.
