Registered Nurse - RN or Licensed Practical Nurse - LPN Hospice Visits

📋 Description

• Home visits for hospice patients in designated territories (North Jersey).
  
• Provide care per visit; meds, wound care, treatments, comfort care.
• Monitor patient conditions and report changes to supervisor.
• Follow Doctors’ orders and document appropriately.
• Travel to cases as assigned; occasional after-hours/weekends.
• Per diem, weekday availability; 1-2 holidays per year.

🎯 Requirements

• Active NJ RN or LPN license.
• Minimum 1 year clinical (medical/surgical) experience.
• Home care/hospice experience preferred but not required.
• Strong computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
• Spanish bilingual is a plus.
