Registered Nurse (RN) - Telehealth Medicare Annual Wellness Visit Specialist - PRN

📋 Description

• Conduct Medicare AWVs via telehealth per CMS guidelines.
• Review and update patient histories and meds remotely.
• Assess health status, vitals, cognition, and risk factors remotely.
• Educate patients on preventive care and community resources virtually.
• Develop and update personalized prevention plans with physician guidance.
• Document and follow up on identified concerns in the EHR.

🎯 Requirements

• Must be bilingual in Spanish and English.
• Must be located in GA with a current, unrestricted GA RN license.
• PRN, Mon-Fri 8am-5pm; ~20 hours/week.
• Minimum 3 years in patient care; primary/preventive/telehealth preferred.
• Experience with Medicare AWVs and telehealth platforms desirable.
• Proficient in EHR and telehealth platforms; strong communication.
