Registered Nurse (RN) - Telehealth Medicare Annual Wellness Visit Specialist - PRN

📋 Description

• Conduct Medicare AWVs via telehealth per CMS guidelines.
• Review and update patient histories virtually (meds, immunizations, family history).
• Assess health status, vitals, cognition, and chronic disease risk via remote monitoring.
• Educate patients on preventive care, lifestyle changes, and resources via virtual visits.
• Develop and update personalized prevention plans with physician guidance.
• Collaborate with supervising physician for accurate documentation and follow-up in EHR.

🎯 Requirements

• Bilingual in Vietnamese and English.
• Located in GA.
• Active RN license in GA.
• PRN availability Mon-Fri, 8am-5pm, ~20 hours/week.
• Minimum 3 years patient care nursing experience (prefer primary/preventive/telehealth).
• Experience with Medicare AWVs and telehealth platforms desirable.
