Registered Nurse (RN) - Telehealth Medicare Annual Wellness Visit Specialist - PRN

📋 Description

• Conduct Medicare AWVs via telehealth per CMS guidelines.
• Review and update patient histories remotely.
• Assess health status, vitals, and cognition using remote tools.
• Educate patients on preventive care and community resources virtually.
• Develop personalized prevention plans with physician guidance.
• Document findings and follow-ups in the EHR.

🎯 Requirements

• Must be bilingual in Korean and English.
• Must be located in GA.
• Current and unrestricted RN license in GA.
• PRN availability Mon-Fri 8:00am-5:00pm (~20 hrs/wk).
• Minimum 3 years of patient-care nursing experience.
• AWVs and telehealth platforms experience preferred.
