Registered Nurse, Senior Living Home Health

📋 Description

• RN to join home health visit team in Senior Living Communities in York County, PA
• Provide skilled nursing and rehabilitative care to help clients remain at home
• Collaborate with a multidisciplinary team and ensure compassionate, high-quality care
• Home visits with acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical
• Independently triage and manage urgent clinical situations in the home

🎯 Requirements

• Current PA nursing license in good standing
• Graduation from an accredited nursing program
• At least 1 year of adult/geriatric clinical experience in relevant settings
• Ability to work independently with strong decision-making and documentation skills
• Comfort with varied home environments and flexible scheduling
• Strong attention to detail and cultural sensitivity
