📋 Description

• Lead field staff in delivering quality client service and clinical care
• Oversee internal case management and staff development
• Ensure adherence to policies and Medicare Conditions of Participation
• Review OASIS forms and care plan coding
• Coordinate interdisciplinary communication and supervision
• Visit clients to assess care and supervise staff as needed

🎯 Requirements

• Current PT or OT license in good standing
• Prior supervisory experience
• Minimum two years in community health
• Experience with Medicare Home Health workflows
• Strong knowledge of OASIS and MCP for Home Health
• Excellent organizational and communication skills
