Remote Clinical Pharmacist – HEDIS Medication Adherence

📋 Description

• Lead HEDIS medication adherence initiatives to improve measures.
• Analyze adherence data, identify gaps, and implement interventions.
• Collaborate with providers, care managers, and stakeholders to promote adherence.
• Conduct MTM sessions, educate patients, and address barriers.
• Use EHR and pharmacy data to identify at-risk patients and plan actions.
• Monitor HEDIS performance and report progress to leadership.

🎯 Requirements

• PharmD degree from accredited school.
• Active pharmacist license in TX, PA, AL, or TN.
• 2+ years in HEDIS adherence work or quality improvement.
• Strong knowledge of HEDIS measures for adherence.
• Proficiency with EHR systems and data analysis tools.
• Excellent communication, problem-solving, and organizational skills.
