Remote Clinical Pharmacist – HEDIS Medication Adherence

📋 Description

• Lead efforts to improve HEDIS med adherence & other quality measures
• Analyze adherence data to identify gaps & implement interventions
• Collaborate with providers, care managers & stakeholders
• Conduct MTM sessions & educate patients on regimens
• Engage patients with motivational interviewing for adherence

🎯 Requirements

• PharmD from an accredited school
• Active pharmacist license in TX, PA, AL, TN
• ≥2 years in HEDIS-related adherence or population health
• Strong knowledge of HEDIS measures for adherence
• Proficiency with EHR systems & data analysis tools
• Excellent communication, problem-solving & organization
• Ability to work independently and in a team
• Experience with motivational interviewing is a plus
