📋 Description

• Cover Transitions of Care for post-hospital patients.
• Telephonic outreach and scheduled video/phone visits after discharge.
• Focus on medication reconciliation, symptom screening, and follow-ups.
• Build trust with patients and explain Galileo's care model.
• Coordinate care across hospital, ED, pharmacies, and providers.
• Collaborate with the nursing team.

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics
• Active state licensure in SC, with ability to obtain additional state licensures
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
• Motivational interviewing skills
• Bilingual in English and Spanish, including cultural fluency
