📋 Description

• Work remotely with medical practice staff to manage target patient population.
• Work independently and as part of the care team to support patient care.
• Help patients achieve better outcomes; connect to care, referrals, and resources.
• Document patient history and issues in the EHR accurately.
• Understand referrals and prior authorizations; navigate care pathways.
• Build trusted relationships with medical practices and Guidehealth while meeting KPIs.

🎯 Requirements

• Certified/Registered Medical Assistant or EMT with 2+ years experience.
• Passion for coordinating care for complex medical/social needs.
• Willingness to learn motivational interviewing and behavioral change.
• Strong interpersonal skills with empathy and professionalism.
• Excellent written/verbal communication; able to document interviews in real time.
• Familiar with EHRs (eClinicalWorks, EPIC, Athena, Cerner) and MS Office.
