📋 Description

• Building meaningful relationships with patients and families, understanding their goals, barriers
• Creating personalized care plans that go beyond diagnoses, addressing social determinants and
• Coordinating care across providers and settings, ensuring safety, quality, and continuity.
• Educating and empowering members to make informed decisions and navigate the healthcare system
• Collaborating with an interdisciplinary team, sharing insights and advocating for whole-person
• Using data as a tool for insight—not the driver of care—to identify opportunities and measure

🎯 Requirements

• Minimum of 5 years as a Registered Nurse
• Active RN license: compact, multi-state licensure
• Strong background in care management as well as complex and chronic condition management
• Skilled in motivational interviewing, patient education, and patient advocacy
• Ability to work independently in a remote environment while staying connected to the team
• Comfort with technology and multiple EMR systems
