🎯 Requirements

• Active Illinois RN license
• Five+ years clinical experience across diverse settings
• Care management and chronic condition management
• Motivational interviewing, patient education, advocacy
• Remote work ability; independent work with team connectivity
• Advanced knowledge in psychiatric conditions and crisis intervention
