Remote Triage, RN - Per Diem

📋 Description

• Answer after-hours calls and resolve issues within scope.
• Coordinate telehealth meetings between participants and clinicians.
• Ensure timely care delivery; escalate issues as needed.
• Troubleshoot transportation, med delivery, and discharge issues.
• Support smooth care transitions between settings (hospitals, SNFs).
• Build relationships with participants and provide ongoing care remotely.

🎯 Requirements

• Graduate of an Accredited School of Nursing with an unencumbered RN license
• Nursing knowledge to treat frail elderly participants and manage complex clinical situations
• Highly motivated, self-directed; able to act quickly in emergencies
• Excellent clinical, organizational, and communication skills with seniors, families, and teams
• Able to work shifts incl days, evenings, nights, weekends, holidays
