🎯 Requirements

• Minimum of 5 years as a Registered Nurse
• Active Compact RN License
• Strong background in care management for complex and chronic conditions
• Motivational interviewing, patient education, and advocacy
• Ability to work remotely and stay connected to team
• Comfort with technology and EMR systems
